Description
Good Connections is a community wellbeing social prescribing service available to people living within the range of West Calder Medical Practice.
The Good Connections service uses local knowledge to help you connect to services and opportunities available in West Lothian that will improve your health, wellbeing and social connections. The Good Connections service is staffed by a Community Wellbeing Link-worker who works with you on a one-to-one basis to identify services and opportunities available in the local area that best suit your individual needs and goals. The link worker will then support your through signposting and referrals to other services.
Examples of the type of the support and opportunities that Good Connections can help you access include:
- specialised support and advice services fif you are struggling with specific health or social issues.
- activity groups to make new social connections in the community, get some outdoor exercise or try something new.
- counselling services or peer support groups if you are struggling with a variety of mental health difficulties
- volunteering opportunities if you are looking to build confidence or learn new skills
These are just a few reasons why you might want to access the Good Connections service. Social prescribing can particularly benefit you if you are struggling with mental health difficulties, feeling socially isolated and if you have a long-term medical condition.
